- Birth Certificate to attest the birth of Michal Chlapec on March 1st 1918, in Hlohovec. Horizontal document, which has been folded several times both horizontally and vertically. Top is written "Rodný list," with a large table underneath where the details are handwritten. Document is signed and dated June 8th 1942. Three fiscal stamps on the bottom center, with a blue image, value of 3ks. Narrative: Donor is the wife of Viktor Matus (born Victor Neumann), who survived the war in hiding and came to Montreal in 1949. From Hlohovec, Czechoslovakia. Two older brothers André and Otto were killed in January 1945 uprising. Parents were deported and killed in Auschwitz. Brother Marcel Neumann survived.

Issued to Michal Novotný, employment : locksmith. Dated November 4th 1941. On back, top right, fiscal stamp with blue image, value "5ks". Inside cover contains a black and white photograph. Fold out page contains a document on light green paper which reads "Výu?ný List" on the top, followed by a paragraph of text both printed and handwritten. Document bares two signatures at the bottom and is dated, 1934. Fiscal stamp at the top left, red with a value of 1 koruna. First page of the work permit is a form filled out in handwriting. There are several pages of printed text followed by a page with a form, which has been filled out by hand in black ink. The rest off the booklet contains pages of blank forms. Narrative: Donor is the wife of Viktor Matus (born Victor Neumann), who survived the war in hiding and came to Montreal in 1949. From Hlohovec, Czechoslovakia.
