1 page, single-sided. Folded once horizontally and once vertically. Printed form filled in by hand with decorative border. The word 'Duplikat' at top right corner indicates that this is a duplicate of the original document. Document is a certificate of affiliation for Sara Herman from the city of Horyhlady, Poland. The certificate attests that this city is her hometown. Stamp at top right corner indicates that Sara Herman was issued a passport at the Polish Consulate in Vienna. Narrative: Sara (Hermann) Heiss was the mother of the donor, Erika Bloom. The Heiss family fled Nazi-occupied Vienna in 1938, after Salomon was arrested and detained in Dachau for 3 days. They survived the war in Shanghai, immigrated to Israel in 1949, and eventually settled in Montreal.
